Case Manager - Registered Nurse
Position Overview:
As a Case Manager RN, you will be responsible for coordinating patient care, ensuring the appropriate level of care, and guiding patients through their healthcare journey. You will assess the medical necessity for admissions, discharges, and transfers, while collaborating with multidisciplinary teams to develop individualized care plans. This is a critical role in ensuring patients receive high-quality, continuous care in a timely and efficient manner.
Responsibilities:
- Assess, plan, implement, and evaluate care for patients across various settings.
- Determine the appropriateness and medical necessity for all inpatient admissions, observation stays, and changes in care level.
- Lead the Transition of Care Team to ensure continuity of care and effective discharge planning.
- Coordinate ancillary services for patients, ensuring all needs are met.
- Verify status orders and ensure accurate documentation in inpatient departments.
- Communicate with healthcare providers and families to ensure patient-centered care.
Qualifications:
- Associate Degree in Nursing (ADN), with current RN licensure in the state of Colorado (or ability to obtain prior to start date).
- Basic Life Support (BLS) certification (can be obtained upon hire).
- Prior case management or hospital experience is preferred but not required.
