Registered Nurse Patient Navigator Opportunity

MLR is looking for a dedicated and skilled Registered Nurse Patient Navigator to join our client's hospital team near Minturn, Colorado!
RN Patient Navigator - Key Responsibilities:
- Facilitate care coordination, supporting patients throughout their healthcare experience, from diagnosis to recovery and follow-up.
- Work hours: 8:30 AM - 5:00 PM.
- Inform patients and their families about medical treatments, procedures, and available support services.
- Collaborate with healthcare teams to create and implement tailored, patient-centered care plans.
- Ensure thorough and accurate documentation of patient care, keeping records up-to-date for healthcare providers.
Qualifications for an RN Patient Navigator:
- Active RN license in Colorado, or eligibility to secure one before starting.
- Associate Degree in Nursing (ADN) is mandatory.
- Basic Life Support (BLS) certification (can be obtained upon hire).
- Advanced Cardiac Life Support (ACLS) certification is required.
- A minimum of 3 years of nursing experience, especially in inpatient or outpatient orthopedic settings.
