Home Health Physical Therapist Position Overview:
MLR is looking for a passionate and skilled Home Health Physical Therapist to join our committed team. In this role, you'll deliver in-home therapy services to help patients improve mobility, strength, and overall function.
Key Responsibilities for Home Health Physical Therapist:
- Patient Assessments: Evaluate patients' physical abilities and limitations in their home environment, developing customized plans to enhance mobility, strength, and functional skills.
- Therapy Planning & Implementation: Design and implement therapy programs to improve movement, balance, and daily function, with a focus on meeting the patient’s individual goals and needs.
- Patient & Caregiver Education: Offer training to patients and their families on exercises, home modifications, and techniques to improve safety and mobility.
- Collaboration with Healthcare Team: Work alongside physicians, nurses, occupational therapists, and other healthcare professionals to provide coordinated and patient-centered care.
- Documentation: Ensure accurate and timely documentation of treatment plans, patient progress, and outcomes in accordance with home health care regulations and standards.
Qualifications for Home Health Physical Therapist:
- Education: Graduate of an accredited Physical Therapy program.
- Certification: Illinois Licensure in Physical Therapy required within 6 months of hire.
- Experience: Experience in home health or outpatient therapy is preferred, but new graduates with a solid clinical background are encouraged to apply.
- Skills: Strong interpersonal and communication skills, excellent time management, and the ability to work independently while providing high-quality care in patients' homes.
