Acute Care Case Manager – RN / Social Work | Full-Time | North Central New Mexico

As a Case Manager, you will work closely with physicians, nurses, and other healthcare professionals to coordinate high-quality, cost-effective care for patients throughout their hospital stay and discharge process. 

Whether you are a Registered Nurse (RN) or a licensed Social Worker (LMSW, LISW, or LCSW), this opportunity offers a chance to make a lasting impact while living in one of the most culturally rich and naturally beautiful regions of the Southwest. 

What You’ll Do: 

• Conduct comprehensive assessments to identify patient needs and develop individualized discharge plans 
• Coordinate services including home health, skilled nursing, rehabilitation, and community-based resources 
• Collaborate across disciplines to ensure optimal patient outcomes and safe transitions of care 
• Advocate for patients and their families by navigating complex healthcare systems and insurance processes 
• Support efforts to reduce length of stay (LOS), readmission rates, and resource utilization 
• Provide timely and thorough documentation in EMR systems according to regulatory requirements 
• Educate patients and families about available services and follow-up care 
• Issue required Medicare notifications and assist with appeals when needed 

Qualifications: 

Education: 

• RN: Graduate of accredited nursing program (BSN preferred) 
• OR MSW: Master's in Social Work from an accredited program 

Licensure & Certification: 

• RN: Active RN license in New Mexico 
• Social Work: LMSW, LISW, or LCSW in New Mexico 
• BLS required 
• Case management certifications (CCM, ACM, or RN-BC) preferred 

Experience: 

• At least 2 years of healthcare experience, preferably 1 year in an acute care or hospital setting 

Skills: 

• Effective communicator and strong interpersonal skills 
• Detail-oriented with strong problem-solving ability 
• Comfortable navigating EMR systems and Microsoft Office 
• Knowledge of insurance regulations and post-acute care options
